Important information regarding the JaxCare Health-Flex Plan - PLEASE READ

If you are a current JaxCare Health Plan member, an employer offering the JaxCare Health Plan, or a physician/anciallary provider offering services to JaxCare Health Plan members, you should already be aware that the Plan will close on June 30, 2008.

During the time leading up to the closure on June 30th, the Plan's operations will continue normally, and the Member Handbook rules and benefits still apply.

Any claims for services provided after June 30th WILL NOT BE PAID. Please arrange for other health care coverage before seeking medical care after June 30th.

If you are not currently a JaxCare Health Plan member but are trying to enroll, the plan cannot accept any new members. The Plan will close completely on June 30, 2008.

JaxCare is working with other health care safety-net providers to develop a coordinated network primary care services for the uninsured. As more details become available, this website will be updated
